Injury Reporting

If a player is injured during a game, practice, or other league-approved activity that may or may not require medical attention, league officials (i.e. manager/coach/designee) should follow these steps:

  • Administer any initial first aid treatment (if necessary).
  • Contact the player’s parent or legal guardian if they are not onsite at the time of the incident.
  • Document the incident with as much detail as possible on the Little League Injury Tracking Form. Paper copies are also available in team first aid kits.
  • Send a copy (or photo) of the form to the SLPLL Safety Officer at [email protected].

Accident Claim Form Instructions

Little League Accident Claim Form

Severe Weather

Lightning
If lightning is visual or thunder is audible, the field must be cleared immediately. SLPLL enforces a strict 30-minute wait rule: Play cannot resume until 30 minutes have passed since the last indication of lightning. The concession stand lead is designated to monitor the lightning detector and notify umpires if nearby lightning has been detected.

Tornadoes and Severe Weather
In the event of a tornado, all games and practices must be cancelled immediately. Managers and coaches are responsible for relocating all team members to the nearest designated safe shelter.

Heavy Winds
All activities must stop immediately if wind speeds reach or exceed 25 mph. Play or practice may only resume once sustained winds drop below this threshold. Umpires and coaches hold the joint responsibility for monitoring conditions and enforcing this suspension to maintain a secure environment for everyone on the field.

Heat Illness Prevention
On days where the Heat Index exceeds 90°F, mandatory water breaks will be enforced every 3 innings or 20 minutes. Iced towels will be available for rapid cooling.

Poor Air Quality
In recognition of the increasing frequency of wildfire smoke events, which pose a growing threat to children's respiratory health in Minnesota, SLPLL has adopted a strict Air Quality Index (AQI) protocol. If the official AQI is measured at 151 or higher (a level designated as 'Unhealthy'), all scheduled outdoor activities, including practices, games, and field maintenance, will be immediately canceled or rescheduled. While mandatory cancellations occur at this threshold, we acknowledge that lower AQI levels can still affect individuals differently; therefore, sensitive individuals are encouraged to participate or opt out at their own discretion during "Moderate" or "Unhealthy for Sensitive Groups" air quality events. The current AQI can be found at: https://www.pca.state.mn.us/air-water-land-climate/current-air-quality-conditions.
